The NKADD is seeking a qualified individual for a full-time Case Manager for the Participant Directed Services program (PDS). This individual will be responsible for the development and monitoring of clients’ budgets and service plans to maintain clients’ services in the community. Responsibilities also involve clinical case management activities, including the development and management of care plans. General case management, documentation, and financial oversight of developed care plans are duties required of the individual in this role. This position works closely with community resources and Medicaid offices in determining program eligibility for client base. Populations served are individuals with physical, intellectual, and developmental disabilities ranging in age from young children to older adults.
Bachelor’s degree in a Human Services field and at least 1 year of experience in the field of health and human services; both education and experience must be validated. Candidates must have a valid driver’s license, reliable transportation, and willingness and ability to complete in-home visits. Must possess solid organization skills, as well as strong analytical, problem-solving and decision-making skills. Ability to collaborate well with colleagues, consumers, and partner agencies a must. Maintaining positive work relationships, as well as a professional demeanor is critical to success in this role. Understanding of software necessary; must have ability to use, and a comfort level with, laptop computers.
