The NKADD is seeking a qualified individual for a full-time Geriatric Case Manager.
Responsibilities include assessing eligibility and providing ongoing case management for state and federal services. These services aid older adults to remain independently in their homes for as long as they can. This is accomplished in part via monthly home visits, regular documentation, and ongoing phone contact with individuals. Services that are monitored can include Home Delivered Meals, Personal Care, Homemaking, Chore and Home Repair as well as Respite services. The position includes the use of a database, requires regular off-site office work/home visits, and the organization and oversight of some events. The ideal candidate will be able to demonstrate high personal integrity, a desire to serve the community, and the ability to work independently as well as part of a team. Bachelor’s degree in Social Work or related Human Service Field, or RN is required. 1-year minimum experience in care of the elderly. Valid driver’s license is necessary to be considered. Reliable transportation and ability to complete in-home visits throughout an eight-county region are also required. Strong organizational skills necessary with attention to detail and demonstrated ability to prioritize responsibilities. Must also possess excellent analytical, problem-solving and decision-making skills as well as sound judgement. Must have the ability to understand and use job specific software as well as a level of comfort with the use of laptop and computers.
Starting salary range from $29,000 to $31,000 DOQ. EOE. TDD 711.
