Case Manager / Service Advisor
The Northern Kentucky Area Development District is hiring an experienced, communicative, and compassionate individual to serve as full-time Rural Case Manager.
Our program helps disabled individuals, including children and adults throughout Northern Kentucky, to remain in their homes through a comprehensive care plan. Assisting these families includes the following responsibilities: functioning as a part of the team by working with individuals and families on the development and implementation of care plans; approval of funds for available services as specified by program regulations; completing monthly home visits to monitor and assess service delivery; assisting in the development of annual cost budgets for services; monitoring and authorizing the amount, scope, and duration of services for individuals and families; and appropriately documenting analysis and decisions for all client and case-related contracts. Our Service Advisors/Case Managers support and assist the people of Northern Kentucky through compassionate care, compliance with regulations and guidelines, and regular follow-up with individuals and families served by the Waiver Program. Requirements
Four-year college or university degree in the field of Human Services such as Social Work, Psychology, Sociology, RN or related degree and at least 1 year of experience in the field of health and human services required. Both education and experience must be validated. Candidates must have and maintain a valid driver’s license, reliable transportation, and willingness and ability to complete in-home visits on a monthly basis.
At this time due to the COVID-19 crisis, these visits are not in-person, however in-person visits will be resumed upon State Program authorization. Must be dependable and possess solid organizational skills, strong analytical and problem-solving skills, as well as the ability to make sound decisions with a compassionate approach. Collaboration with colleagues, consumers, and partner agencies a must. Maintaining positive work relationships, as well as a professional demeanor is critical to success in this role. Understanding of software necessary; must have ability to use, and a comfort level with, laptop computers. Applicants must successfully pass a background check and pre-employment health screening (urine drug screen and TB test).
Starting salary range from $32,000 to $35,000; Excellent benefits and generous retirement plan. DOQ. EOE. TDD 711.
