Case Manager

The NKADD, is seeking a qualified individual for a full-time Case Manager. The role of this Case Manager is assisting older adults to remain independently in their homes for as long as they can by managing plans of care, while fostering strong lines of communication with clients/consumers. The following responsibilities are included: assessing eligibility for state and federal services and providing on-going case management; conducting monthly in-home visits to establish and review care plans; monitoring receipt and quality of services for clients/consumers (monitored services can include Home Delivered Meals, Personal Care, Homemaking, Chore and Home Repair as well as Respite services); conducting monthly phone inquiries to discern issues or concerns; identifying needs and completing follow-up with clients/consumers and/or partner agencies for service coordination; and timely completion of required documentation ensuring compliance with and adherence to regulations.

This position does necessitate the use of a database, regular off-site home visits, and the ability to communicate with respect and compassion. The ideal candidate will be able to demonstrate high personal integrity, a desire to serve the community, and the ability to work independently as well as part of a team. Exceptional communication, organizational, and writing skills as well as knowledge of community resources are critical in this role

Requirements
Bachelor’s degree in Social Work or related Human Service Field, or RN is required. 1-year minimum experience in care of the elderly. Valid driver’s license is necessary to be considered. Reliable transportation and ability to complete in-home visits throughout an eight-county region are also required. At this time due to the COVID-19 crisis, these visits are not in-person, however in-person visits will be resumed upon State Program authorization. Strong organizational skills necessary with attention to detail and demonstrate. ability to prioritize responsibilities. Must also possess excellent analytical, problem-solving and decision-making skills as well as sound judgement. Must have the ability to understand and use job specific software as well as a level of comfort with the use of laptop and computers.
